How to book your driving theory test
Before applying or booking driving theory test you must be a holder of a valid provisional driving licence. At the age of 17 you can apply for a provisional driving licence from the Driver and Vehicle Licensing Agency (DVLA). Provisional application forms (form D1) are available with your local Post Office. You can book theory test once you became the holder or have a valid provisional driving licence. If you are a holder of a valid driving licence which is of car and you want to learn to drive a large vehicle, minibuses or buses you have to apply for provisional entitlement for those vehicles.
Driving theory test appointments can be booked, changed or cancelled in three clear working days notice by using driving test booking online service or telephonically. Once you become a holder of valid provisional driving licence you can book driving test. Waiting times differs from region to region, but the target of that is to provide an appointment date within two weeks of their preferred date to the 95 per cent of theory test examinee.
Through the Driving Standards Agency (DSA) you have book the theory and driving practical test (car practical test, motorcycle practical test, minibuses or buses).
There are various ways through which you can book your self for driving theory test. They are as follows –
- Online booking
- Booking by post
- Booking driving test over phone.
Online booking
If you want to book or apply yourself for driving theory test online you will have to provide -
A valid UK driving licence A valid debit or credit card for payment (Visa, MasterCard, Delta, Visa Electron, Switch/Maestro and Solo cards are accepted)
Booking by post
An application form has to be filled if you want to book theory test by post. Application forms are available online or on request from the Driving Standards Agency (DSA) booking line. Application forms for the theory test should be sent to the address shown on the form. The address on these forms is

Cash payments are not accepted. Payment can be made either by cheque or by postal order.
Booking driving test over phone –
You can book a theory test over the phone using the numbers below, from 8.00 am to 4.00 pm except on Bank Holidays

While booking for a theory test on phone you will need –
- A valid UK driving licence

Cancelling or rescheduling a test
You can reschedule or cancel your respective test either telephonically via the booking line or through online for which you need to give DSA three clear working days notice through which you can:
- Cancel your appointment
- Move or change your appointment
- Have a full refund of test fee

FDA Alert - Vaginal Mesh
On July 13th 2011, the FDA issued a safety communication regarding 'Serious Complications Associated with Transvaginal Placement of Surgical Mesh for Pelvic Organ Prolapse'.
In other words, serious complications have been linked to the use of vaginal mesh or transvaginal mesh. As discussed previously, this vaginal mesh is an implant used to strengthen vaginal tissue that can weaken due to childbirth, hernias, etc. According to the FDA, the purpose of their "serious concerns" are specific to pelvic organ prolapse (POP) and stress urinary incontinence (SUI).
The most common complication that was reported for these vaginal mesh implants was an erosion of the device through the vagina, also known as a protrusion. The vast majority of these reports came between 2008 and 2010.
Some of the most important revelations from the FDAs investigation (taken directly from the FDAs website) are as follows:
-
- Mesh used in transvaginal POP repair introduces risks not present in traditional non-mesh surgery for POP repair.
-
- Mesh placed abdominally for POP repair appears to result in lower rates of mesh complications compared to transvaginal POP surgery with mesh.
-
- There is no evidence that transvaginal repair to support the top of the vagina (apical repair) or the back wall of the vagina (posterior repair) with mesh provides any added benefit compared to traditional surgery without mesh.
-
- While transvaginal surgical repair to correct weakened tissue between the bladder and vagina (anterior repair) with mesh augmentation may provide an anatomic benefit compared to traditional POP repair without mesh, this anatomic benefit may not result in better symptomatic results.
Patients that experienced either mesh erosion or mesh contraction reported severe pelvic pain and/or discomfort.
If you have had a vaginal mesh implant and have suffered from any discomfort, it is important that you contact your physician immediately. There are surgical steps that can be taken to repair this damage and/or reduce the pain or discomfort. If you are interested in legal remedies from such complications, it would be best to speak with a vaginal mesh lawyer.
If you have not had a mesh implant but are considering doing so, please be aware of the alternatives (such as biologic grafts) as well as asking your physician the appropriate questions (recommended by the FDA):

- Are you planning to use mesh in my surgery?

- Why do you think I am a good candidate for surgical mesh?

- Why is surgical mesh being chosen for my repair?
-
- What are the alternatives to transvaginal surgical mesh repair for POP, including non-surgical options?
-
- What are the pros and cons of using surgical mesh in my particular case? How likely is it that my repair could be successfully performed without using surgical mesh?
-
- Will my partner be able to feel the surgical mesh during sexual intercourse? What if the surgical mesh erodes through my vaginal wall?
-
- If surgical mesh is to be used, how often have you implanted this particular product? What results have your other patients had with this product?
-
- What can I expect to feel after surgery and for how long?
-
- Which specific side effects should I report to you after the surgery?
-
- What if the mesh surgery doesn't correct my problem?
-
- If I develop a complication, will you treat it or will I be referred to a specialist experienced with surgical mesh complications?
-
- If I have a complication related to the surgical mesh, how likely is it that the surgical mesh could be removed and what could be the consequences?
-
- If a surgical mesh is to be used, is there patient information that comes with the product, and can I have a copy?

Micromanagement
This is not the first time I have talked about micromanagement over the years, and I am sure it won't be my last. Recently, I had some business friends complain to me how their employees cannot follow directions. But on the other hand, I also know a lot of people who wonder why management doesn't trust them to do their job properly. You see this not only in the corporate world but in nonprofit organizations as well. Today, managers are spending more time supervising the work of others as opposed to actually managing them.
Back in the 1960's and 1970's we talked a lot about empowering workers and teamwork, but the pendulum seems to have swung the other way and micromanagement is now in vogue in today's corporate cultures. I have a theory as to why this has happened:
First, we now live in a litigious society where everyone is paranoid about accepting responsibilities that may result in a lawsuit. As a result, employees come down with an acute case of "The Stupids" and heaps everything on their manager's desk. Such a mindset means there is little, if any, self-initiative by employees.
Second, we overly structure the activities of our youth, be it at home, in school, or on the playground. For example, when I was a kid I was always ready for a pickup game of baseball (I think I carried my glove and bat with me just about everywhere). But the youth of today doesn't think this way anymore. Instead, they need uniforms, equipment, coaches and manicured baseball fields in order to play. Further, they are more inclined to play an electronic game indoors as opposed to interacting with their peers. This is causing our youth to become socially despondent and a legitimate cause for concern in the workplace in the years ahead. And because they are only being given tasks to perform around the home, and not responsibilities, there is no sense of initiative being instilled in them. In other words, our youth are being subliminally trained to accept micromanagement. How about delegating some responsibilities to them instead? We used to call this "chores" in the old days.
Third, We've forgotten how to manage. Regardless if you are in the corporate world or a nonprofit volunteer organization, our leaders are now more driven by ego as opposed to a results orientation. Being a manager is not about having a fancy job title or building an empire, its about producing a quality product or service on time and within budget. And the only way this can be accomplished is through people. Consequently, managers need to develop their interpersonal communications and leadership skills. Its not about numbers or technology, its about people.
Managers want workers to show some self-initiative and perform their work well, but to do so, you have to train them properly and trust them accordingly. This means building loyalty and investing in the staff. It also means empowering them with responsibility and holding them accountable. Employees have to understand what their duties and goals are, and be allowed to try and conquer them. "Empowerment" implicitly means a worker has a right to try. This of course means motivation, training, and experience.
The three "top-down" primary duties of a manager are:
1. Delegate - prioritize and assign tasks to qualified employees.
2. Control work environment - minimize staff interferences and provide a suitable workplace to operate with the proper tools to perform the work.
3. Review progress - study employee reports and take corrective action where necessary.
In return, the "bottom-up" responsibilities of the workers include:
1. Participate in the planning process - review work specifications and give feedback; estimate amount of time to perform an assignment, assist in the calculation of work schedules with management.
2. Perform work within time and costs constraints.
3. Report activities to management - including the use of time, interferences, possible delays, and anticipated accelerations of schedules.
This "bottom-up" approach to management represents an empowerment scenario where the workers are made to realize their voice is important, builds trust, and encourages initiative.
But if you are the type of manager that finds its necessary to supervise the actions of your workers, than you are part of the problem, not the solution. Remember: "Manage more, supervise less.
